This is a 2006 Kohler bath faucet and I want to replace it. There's a post here with one similar but not the same-the other one has a way to take off from the top, mine does not, see pics. Maybe just use pliers and twist the nut even though the plastic is there?

Depending on the manufacturer, the stem should be a free replacement. It's fairly trivial to replace.
- Turn off water. (IMPORTANT!)
- Remove the large hex fitting on the face of the controller
- With vise grips, pull and wiggle the stem out.
If it's a common manufacturer, you should be able to exchange the stem at your local big box store or plumbing supplier.
Reinstalling is simply the reverse.
While picking up the stem, get an appropriate bolt to fit, and you're good to go.
Caveat: Figure out the manufacturer and call first.
See the threaded tube? There's a nut threaded on that, which holds the plate in place.
Use a Basin Wrench (or any wrench you can fit in there) to loosen the nut (rotate the nut anticlockwise to loosen it), and the plate will slide out.
If the nut is stubborn, and doesn't want to budge. A good shot of penetrating oil can loosen things up.

Obviously, you first remove the stopper linkage, then unscrew the toggle nut. The fact that the large washer has two tabs that project down through the toggle nut makes me wonder, but I would go ahead and twist the toggle CCW and see what happens.
