If you want the fan and light to operate separately, then connecting the wires as you described is the way to go. If you want the whole fixture to be controlled from a single switch, the manual is describing the proper wiring.
As for the green grounding wire, simply attach it to the green grounding wire in the ceiling. You should use an appropriately sized ring or fork terminal, to connect the ground under the same screw as the ground from the ceiling.
The green wire is a ground wire. The purpose of it is to cause the breaker to trip or the fuse to blow if a short circuit develops. The majority of the time you can just hook up the white (neutral) and black or blue wires and never have any problem.
HOWEVER! If a short circuit does develop you may burn the place down!
If you have a metal box connected to metal clad flexible conduit or EMT you should be able to screw a #10 screw into the box and fasten a green wire to it completing a ground. Whatever you have, there should have been a bare copper wire coming in to the box with the black and blue. Some do it yourselfers figure they aren't important and cut them off.

The manual for your fan should outline instructions for this type of wiring setup, but generally this will hold true:
Black in box goes to blue & black in fan.
White in box goes to white in fan.
Connect all grounds together.
This will cause your switch on the wall to control both the fan motor and fan lights. You'll have to use your pull chains to control the two independently.
