I am putting a basement bath in and I need help. I will be adding a new 20 amp circuit for the single pole outlet. Also, a 20 amp line for a 3 switch box. Switch #1 is for the vanity light, #2 is for the exhaust fan light and #3 is for the exhaust fan.
I would appreciate a drawing showing from the 3 gang box the wiring of the circuit.
Something is wrong with the wiring to the ceiling fan or with the connections to the switch.
Possibly in between the last succesful test and the first unsuccessful use of that switch something has damaged the wiring (e.g. a nail into the wall? strain on the wires?)
I would isolate the circuit at the breaker box, test for voltage at the switch and then test the wires attached to the fan switch for unexpected continuity between live & neutral, live & earth. I'd then detach the switch from all wiring and test for continuity across it's contacts with the switch in each state.
You can do it using only 12/2 and 12/3 cables like this.
Or you could use 12/2 and 12/4 cables like this.
Because there will be so many wires in the box, you'll want to get at least a 34 cu. in. double gang box. Like this one.

The following diagram should help you to visualize how to wire up the three switches. In your case you may have 12 AWG wire due to the 20A circuit. Also you will substitute one of the lights with the blower fan motor connection.
