After removing the carpet from our bathroom closet, I found urine stains on the plywood sub-floor (serenity now).
It sounds bad but the floors are quite sturdy so I'm reasonably certain that it's a superficial stain. What would be the recommended way of cleaning it (short of removing/replacing the sub-floor).
I'm planning to eventually place carpeting back in the closet so I'm not concerned with leaving a mark. Just want to make sure it's sanitary and odor-free.

I have used that mixture with good success. Make sure to wipe down everything that the dog might have come in contact with, and I mean everything. Floors, walls, trim, doors, etc. You might need to repeat the procedure a few days later.
The skunk odor tends to linger quite a bit and you will find that it gradually reduces in intensity over time but is still evident. Anytime your dog gets wet you smell it, likely for months!
If it is really bad and does not go away you might opt to bring in a professional cleaning company who will have stronger chemicals to deal with it.
From a comfort and water protection perspective, I think you'll find it beneficial to install a sub floor. I'd recommend one which allows for ventilation beneath it such as these 2x2 panels you can buy at Home Depot/Lowes
This will give you a nice, warm "softer" surface to put your carpet pad on, and help protect from any condensation issues with the cold floor beneath by allowing it a place to form, and then with the proper ventilation openings cut into the subfloor (standard floor vent in the carpet to allow airflow) it'll dry out on its own.

I use Zep pet spotter, but it's intended for carpets, not wood. I think mop it with a dilute solution of chlorine bleach to sanitize the area, then seal it with a primer / sealer to prevent any residual odors from escaping.
