I'm not sure how much room you have to work in, but an option would be to rent a 16 foot step ladder at your local tool rental center.
If the bulbs are pointed down, maybe some type of bulb grabber on an extension pole.

It looks like it just pulls down, there are usually little springs that keep it in place. Like the one pictured below

It should unscrew counterclockwise if you put friction on the inner chrome ring with your fingertips.
If it does not unscrew, you can pull the whole thing out of the receiver by using your fingernails on the outermost edges and pulling down. You can then inspect it for how to remove the lamp.
