I want to mount shutters on my 1930's house, but since we've had it insulated with polystyrene, the recess is 7 inches. That would need a huge offset hinge!! The windows are metal-cased woodframed double glazing. I would like the shutters to look authentic, and don't want to mount them outside the window. We would use Fischer Thermax wal plugs for the outside mount, but my concern is the window frame, and getting hold of such long shutter hinges.

If you want to use a screw I would use Tapcon® Concrete Screws. They hold very well and are fairly easy to use. The only draw back is that they are a bright blue and you will have to countersink them and caulk or plugged the hole. 3/16 inch will be strong enough, you will want about 1.5" of penetration. To install you just drill a hole matching the size of the screw (needs to be exact size required) and screw them in. Using a screw is helpful because its about the only concrete anchor you can easily remove.
The other option is Metal Hit Anchors. These are a nail in a sleeve. you pre drill the hole and hammer the anchor in and the nail expands the sleeve.
Any time you drill into masonry you should be using a hammer drill, and a masonry bit that is specifically designed to be used with a hammer drill.
On most brick homes anchoring into the brick is going to be stronger. But it is only shutters and you could get away with drilling into the mortar. This way if you ever want to take the shutters down you can fix the hole in the mortar, unlike if you were to drill directly into the brick.
I used to own and run a Blind business. Living in Florida, a majority of our volume were shutters. I would suggest getting a staple that is slightly longer than the one that came out and then pushing the new one in.
I would protect the lever with a towel and then use a wide wrench, like a plumbers wrench, to squeeze the staple in. Another option which would probably be easier would be to use the same size staple that fell out and use a two-part epoxy to set them in the holes. You may have to get a small syringe to inject the mix into the tiny holes before inserting the staple.

If you want the open shutters to run parallel to the wall, some kind of offset hinge is needed. But creating such a hinge to mount the shutters shouldn't be too hard.
The pivot point needs to be slightly outward of the midpoint of the well around the window. The lower tail of the hinge in the image can be cut off, or it can be bent to fit against the window casing.
You should be able to find standard flat strap hinges that can be bent and cut to suit. A bench vise would be the easiest way to make the bends.
Because there is such a long reach, you may want three hinges per shutter.
