I am trying to find a 125V female plug receptacle with ground for a 12-gauge wire run in the 1960s, the ground wire of which is a much narrower gauge than the white and black conductors. The Levitons and the P&S's at the big box stores assume the three wires are going to be the same gauge, and so they don't get a tight grip the ground wire; it just slips right out. Is there a name for such a connector

No way from here to know which wire fed the light or fan, but you have a continuous hot feed into the box that fed the receptacle, light and fan. The third wire, maybe it fed the fan or maybe something something else farther down the line, if the latter that needs to be tied in 100 percent to the hot feed. All whites still tied together, blacks to the dimmer from the light and hot. All grounds tied together set with a screw to the metal box. Securing the ground to the box is not needed if the box is plastic.

I'm guessing that you are repairing or replacing the light fixtures plug? And if so you would like to know how the wires are connected? The lamp cord as you noticed, has a hot side (which is marked with 'ribs' on the plastic insulation. The other smooth side is the neutral. The repair plug you have is a 2 prong (male) type plug? Which means there is no ground for the fixture. The white wires at the fixture are twisted together and so are the 3 black wires. In order to connect the plug you must connect a white length of wire (and the same for the black) to the wires at the fixture. Twist them tight and secure with a wire nut. You should now have two wires connected to the fixture. ******Please read****: This answer is assuming a lot from your question. Go ahead and buy the modern version. To make the light gauge ground wire secure, add another bit or two of wire—each an inch or so long—beside it into the terminal.
This will fill up the slot enough so that the screw meets the wires and can clamp them together firmly.
