Replacing kitchen light, house wiring to box: 2 white twisted together (neutral), 1 red (hot when switch is on), no ground. Fixture: 1 D shaped ridged wire, 1 smooth (round) wire, 1 ground wire. My guess on connections, D shaped fixture wire to white house wires, smooth (round) fixture wire to red house wire, fixture ground wire to metal fixture box? Is this right?

I suspect the white wire with the stripe is actually the switched power from the switch. Putting together what you have said, I think your setup probably looks something like this:
Where the green wire is the ground and the red wire is your wire with the stripe.
If this doesn't match up with what you are seeing, then let me know what looks wrong and I will try to figure out how it is setup.
The instructions for the fixture are only correct for a metal box. If a metal box was used, the box itself would (should) be grounded. The bracket that holds the light would then be connected to the box, which would make the bracket grounded. Finally the ground wire from the fixture would attach to the bracket, grounding the fixture.
In the case of a plastic box, the box is not grounded. All ground connections must be made by connecting the ground wires together. So in your situation you are correct, you'll connect the ground from the supply to the ground from the fixture using a twist-on connector (or other approved connection).

When a light fixture has two wires of the same color it usually means that polarity isn't important. However, the ridges/ribs may indicate a neutral. Go with your plan.
