Without is a preposition.
Paying is not a verb, but a verbal. The entire phrase paying the bill I believe technically functions as a noun phrase.
Without paying the bill doesn't really answer the question "How did she leave?" so it's not adverbial. Without paying the bill does not really have anything to do with the manner or way (this is what how means) in which she left - it describes an additional and separate idea (it's more of a what). She left AND she didn't pay the bill - she didn't leave in manner or way that obstructed her from paying the bill.
I think the preposition you want to use is on.
To be at or by someone's side usually means a close, physical presence. It conjures up images like this one, where three friends are walking side-by-side:
To be on someone's side, though, usually means to be siding with them in an argument or dispute, or maybe supporting them in a tough situation.

We normally speak of someone being in a photograph rather than "on". What may change things slightly here is that you are referring to a particular part of the photo, the left. We always say "on the left", never "in the left", as that is simply incorrect.
What you see as the left side of a photograph isn't technically the left. The person to the left in a photo was standing on the right when it was taken! So if you are describing a person's position in a photograph technically I suppose you should say "on the left of the photo", because you are referring to the printed photograph rather than what it depicts.
