I was doing a BASEMENT run and I got the D6, D20, and D4. Before breakfast, I used the D4 a few times and after a few more rolls (D6+D20 rolls), I got breakfast. I went to the next level and to my surprise, there were still items that weren't breakfast. I even picked up the small rock, which I distinctly remember picking up earlier in the run. Could the small rock have been put back into the item pool by the D4?

Nope, they don't sadly.
I know because I've tried it, was getting nothing but Breakfast which meant I had exhausted the entire pool. But none of the items that I needed to find were in the collection page.

Meaning that he hasn't unlocked The Lost items (The Soul, The Body and The Mind) but they are shown in his collection page. So it appears to be true.
No. There's no way to "upgrade" from generic treasure item pool to angel or satan items via exhaustion. The quoted passage refers to rerolling i.e. in an Angel Room.
Since the Angel Room Items are replaced with Items from the treasure item pool when exhausted, the special-reroll-condition will likely bring lots of normal Treasure Items before showing the Sacred Heart.

Apparently the D4 (and the D100) will (try and) only replace items with other items in the same item pool, and the small rock is found in no pool whatsoever:
Hence, you can never get this item by D4 (or D100) rerolling.
