From what I know this could be related to how the "spectator" works in game engine - f.ex. join the game as spectator and follow some player - when he reloads his weapon sometimes You won't notice any animation - just played sound - this probably works the same way... I do not have a source for this, but read it somewhere on the steampowered forums...
As you said,by default you should be able to vote maps with number keys(1-8).
In game those keys are used to select your weapons,and as such can be binded to other keys.
So if you change the number 1(primary weapon) to lets say letter K...If you wanted to vote for the first map you will have to press K,pressing 1 wouldn't give your vote.
This is the default binding,if you want to vote maps with numbers 1-8 you should bind it correspondingly.

What's much more important is the above process works on a BOT. We've all had that moment where the bot survives with an AWP while everyone else is broke with 1150$. The bot can be kicked following the above process.
