In my Completed tab, I had a Purchase Refund. I did not get a failed purchase. Where did this come from?

Error 37 is "Servers are Busy", please try again later. It starts popping up when too many people try to log onto the Diablo 3 servers at once.
It's notable because Blizzard specifically warned users that they might run into it at launch, and told them to ignore it and try again.

Yes. In Bastion's Keep Stronghold the merchant "Gorell the Quartermaster" will sell the Plans to create the staff during the final quest in Act 4 "Prime Evil". He is right beside the healer NPC.
As pointed out by Blem, this only works for the improved version of the staff. The normal plans have to be dropped from Izual.

You can get purchase refunds for two reasons:
If you're buying a commodity, and the auction house found someone had posted a cheaper stack up there compared to what you had looked like paying.
Someone else purchased the item you wanted to purchase, so the auction house refunded the amount for the items that you did not receive.
Basically, these are due to trades made between the time you saw the price of whatever you bought in the AH and when you clicked the buyout button. Since the AH does not automatically refresh, there could be a significant difference between what you can see and what's available (though you can get a different "That item is already bought" error depending on the timing).
