When a Demon Hunter is using a crossbow, he can use a shield or even another crossbow in his off-hand at the same time. But when he equips a bow, the shield is removed, and the spot for off-hand item show a quiver for arrows.
What item can I use in my off-hand?
This proc does not refer to which weapon is equipped, rather it refers to the type of skill you are using.
I used this passive while having a crossbow equipped and it would still proc. Also, it procs on any skill that requires "firing your weapon", which are all skills in the Primary, Secondary, and Archery categories.
You cannot gain set bonuses for wearing 2 of the same set item because you would be unable to equip them. Set items are unique equipped as shown in the picture. You are unable to equip two of the same one.

You can use quivers, which are Demon Hunter-specific items.
Unlike in Diablo 2, you don't need bow ammo. Now the quivers are a way to provide additional character bonuses—most quivers come with a damage boost and other magic item properties.
