From my testing, there is no other way to effectively close the portal besides interrupting the cast and/or destroying the treasure goblin. In Hell mode, all enemies have increased resistance and increased HP. Therefore, you HAVE to output more DPS to prevent him from getting away.
From what I have noticed, once his portal is down, you have a set amount of time to kill him or make him run away from the portal before it doesn't matter: He will get into the portal and your loot is gone.
A few recommendations:
- Try cornering him. This will obviously try to prevent him from running away and allow you (and your team if applicable) to dump all of your DPS in a concentrated area.
- If you have a mercenary or other teammate(s) with you, attack him from both sides - in attempt to not allow the treasure goblin from running away.
- If you have not attacked the treasure goblin yet, DON'T ATTACK HIM UNTIL YOU CLEAR THE SURROUNDING AREA. He will only run away once you start attacking him and if he, you, or any of your teammates aggro more enemies on the harder difficulties - it will get harder -practically exponentially per enemy - to take him down before he gets away.
- Lastly, and obviously, make the treasure goblin your party's main priority [if applicable]. Notify everyone in your party that you see a treasure goblin and wait until everyone is ready to take him down (and the area surrounding him is clear).
Nephalem are the children of mixed parentage - angels and demons who created and fled to Sanctuary to avoid the perpetual war between heaven and hell. "Sanctuary" is the primary place where the Diablo games take place, and all of the humans are descendants of these first Nephalem. True nephalem are able to tap into their supernatural powers. Not every "human" in Sanctuary can tap his or her supernatural powers, but the heroes in the Diablo series can.
The paper manual that comes with the boxed copy of the game has a bit of backstory/explanation:
In the midst of this battle, a group of rebellious angels and demons sought refuge. [...] The children of these renegades were known as the nephalem, and they possessed a power greater than their parents could have imagined. [...] As generations passed, the nephalem forgot their heritage. They called themselves human and built their cities, nations, and armies without knowing their true potential.

After he gained a following, Inarius stole the sacred Worldstone and used it to create and conceal a world he called Sanctuary. Demons and angels alike fled there, and some of them—including Inarius and Lilith—fell in love. Many of these unions resulted in children. They were called the nephalem.
Inarius was alarmed when he realized that the nephalem had the potential to surpass both angels and demons in power. He wished to limit their abilities, but Lilith demanded that they become her army. Their escalating conflict led to Sanctuary's discovery by the demon lords and the Angiris Council.
Itherael mentions the Nephalim in "The Destiny of the Nephalim"
Angels and demons are bound to their natures. Demons are given to chaos and deceit; angels to truth and order. [...] The nephalem alone have choice. [...] These creatures dwell outside the order... They cannot fathom the freedom they possess.

Development Hell is a rare Random Dungeon that can be found in Act I. You need to have access to the cemetery, so the minimum requisite is the Shattered Crown quest. If the game you're playing has rolled for the dungeon to appear, you'll find 4 crypts opened in the cemetery, instead of the usual 3, as you can see from the following screenshot (the crypt is clearly marked as "Development Hell").
As you can see from the video you've linked, you don't need to kill Leoric(they're still searching for the chancellor's altar). Also it looks like a single player game, but it can also happen while in a party,as you can see from this image:
There have been various reports of its occurrence in Nightmare, Hell and Inferno difficulties. It still has to be found in Normal, but my guess is that it cannot happen at that difficulty.
