During heart events, residents of Pelican Town give us response options that often vary widely in tone. I know these choices can change dialogue at later heart events, but beyond that I’m curious of their results. Do these choices affect friendship, or have any other in-game effects?

As DanmakuGrazer said, by increasing your relationship with your spouse, it will eventually reach 13 hearts. When this happens, the relationship becomes "capped" like every other relationship: it won't naturally decay. (It can still decrease due to actions such as the character witnessing you digging through trash, or if you give gifts to bachelor/ettes while married.)
Getting your spouse's relationship to 13 is a requirement for
getting a stardrop. More accurately, once your spouse is at 13.5/12 hearts, you'll receive the stardrop. (H/T @Pathoschild.)
In fact, in the past, instead of being represented as 13/12, this would be shown
as a stardrop icon.
It looks like simply talking to them will give all 20 Friendship points, even if you don't finish the conversation.

Just the first thing. As long as you interact with them to get the dialogue box up, you get the points.

So it's apparently either +10 or +20. Any info on why? Any speculations? Any chance it could have something to do with whether or not you "finish" their dialogue for the day (for example, talking to them once when they have multiple dialogues gives +10, but talking to them and finishing the dialogues gives +20)?
It appears that this has been figured out as the wiki now explains the reasoning behind differing Friendship point gains:
talking to villagers (+10 if the villager is in the middle of a special animation, like exercising or using a camera, otherwise +20; can only score once per day);
Thus, as long as you converse with a villager at least once a day and they're not in the middle of something special, you'll get +20 Friendship points, even if the conversation could have kept going.

Yes, some of the dialog choices affect friendship. The random questions are usually less important, but most can give at least a small boost. Some also lose relationship points if you are a jerk.
