Well, I always thought it wasn't reachable since it's inhabited by the tengu, blocked off by the Dominion of Winds. But when I browsed through the explored map on the Guild Wars 2 Wiki, I found the area. I had to zoom in a bit, so it's a bit unclear, but I thought by myself, why would they make a map which isn't explorable? Also, some isles (excluding Claw Island) underneath it seem to be reachable. Does anyone know more about this?

Yes, they are all available. The POI you're missing is called the Lost Grotto. It's tricky because the entrance to the area is across the map. I've marked the area you need to start with a red X. Head to those docks and then in between them, underwater, you'll find a tunnel.
Swim through that tunnel and follow the way through across the zone to find the Lost Grotto.
It's a secret base for the Order of Whispers.

The Vista you are missing is another one that is tricky to get. Take the Farshore Waypoint and run up to Tokk's Mill. At the door, you need to jump on on the straw piles to the roof, then drop down behind them. There's a tunnel/door hidden by some hanging vines to the right after you jump down. You'll need to climb some more rocks and then you'll see the Vista.

That particular Point of Interest gave me some trouble as well. Luckily, one of my guild mates helped me out.
There is a cave on the adjacent island that you must swim into to reach your point of interest. It should be in the general vicinity of the heart on that same island, as illustrated by this freehand circle.

That area is not currently in the game. If it was, you'd be able to find it in the wiki fairly easily.
