When you use summoner heal, I know you get a status effect that reduces subsequent summoner heal effects, but by how much? and does it reduce healing effects from champion abilities/lifesteal/etc?

Currently the Answer is NO.
Once you click on "Save and Use" the masteries are saved to mastery page number X (Where X = 1-10).
I completely understand what you mean with using different summoner spells on the same champion role, so what I created was, different mastery pages for the same role each with a different combination of summoner spells, so for instance my mastery pages looks like:
The one last mastery page is left as a Configurable one during champion selection, so if someone else on my team has clairvoyance and I am playing a support champ, I can get instead of C.V flash or heal.
10 Mastery pages should be enough to customize your preferences.

especially since I then have to remember which one is "Mastery Page 4"
You can change the mastery page name by simply clicking the default name and when done hitting ENTER.
The Spell Vamp portion of the LoL Wiki and my own experience show that
[Spell Vamp] works off of activated item effects, summoner spells, and abilities, regardless of the type of damage done.
Therefore, items that give proc effects to attacks like Bloodrazor, Wit's End, Malady, Wriggles etc, will NOT grant the user health through spell vamp.
On a side note, Ezreal's Mystic Shot (Q) applies on hit effects, including but not limited to, lifesteal.

Heal applies a 35 second debuff that reduces all subsequent Summoner Heal spells, aside from your own, by 50%. This debuff does not affect Lifesteal, Spell Vamp, Health Regen or Champion Heal spells but only other allies' Summoner Heals.
