I recently bought the Mass Effect trilogy and when I redeemed it to my Origin Account, Mass Effect 3 appeared twice. In normal version and an EN Only version. What is the difference between the two?

As I now bought the game I can confirm that the german retail version contains english voicovers and everything. Just select "English" as language when the installer starts.
It depends on what you mean by "in-game". You cannot get to them while actively playing as your character, but you can get to them from the main menu when you start the game. They're under Extras->Accomplishments.

Judging by the name, the EN only version only supports the English language.
