I am trying to create an Iron Golem trap as outlined in this wiki entry (Iron Golem), however they seem to be respawning very slowly. I have enough villagers to spawn at least one golem, what else determines their spawn rate?

You will need to return at least 2 villagers. Then they will start repopulating the village. As of 1.2, villagers will mate and repopulate based on the number of available houses. See this post (especially the "your village died out" section) for more information.
As of the 1.8 snapshots, though, the villagers need to be "willing" to breed in order to breed. It seems that you can make a villager "willing" by trading with them. Willingness is granted on the first new trade offer, and at a 1 in 5 chance per subsequent trade. Once a villager is willing, they will immediately seek another willing mate, if the village can sustain more villagers (based on the number of houses).
12 doors on a side with 4 villagers per side. The vilage 'center' is the middle point between the the 'doors' that are considered to be part of the village. The golems will spawn in the middle and fall thru. DocM has a nice tutorial on youtube describing the mechanics in detail. 20x20x8 footprint for the main cells.
It could be that your layouts dont lend themselves to having a natural 'middle' Even the 2nd picture with the doors all clustered together in the middle, there might not be room for the golem to spawn because the doors are there.

The wiki states that they spawn near houses, and near 15 villages.
Increasing the number of villages will increase the maximum number of golems in your trap, so it will allow the game to try and make a spawn even if there is already a golem in the trap.
Houses are marked by doors, to adding more doors to the trap will increase the number of successful attempts to spawn.
You could also speed up the trap, so there are more spaces for golems to spawn.
