In Minecraft, I'm in survival with a wooden pickaxe, and whenever I mine the iron ore I need, it keeps disappearing. Why is this happening? Am I doing something wrong?

Wooden picks aren't strong enough to mine iron, you need at least a stone pick to do that. All you ever wanted to know about picks.
I tried several commands but i could not find a way to check if a item was not renamed.
You can check, if a pickaxe was crafted:
Create a scoreboard:
/scoreboard objectives add craftedPick stat.craftItem.minecraft.wooden_pickaxe
Then the following commands in a chain:
/give @a[score_craftedPick_min=1] minecraft:wooden_pickaxe 1 1 {YOUR_TAGS}
/scoreboard players remove @a[score_craftedPick_min=1] craftedPick 1
/clear @a minecraft:wooden_pickaxe 0 1

You mentioned you had a wooden pickaxe, this will not work for Iron Ore.
To quote the wiki:
In order to get the Iron ore as an item drop you need to make a stone pickaxe. You can do this the same way you would make a wooden pickaxe just with stone along the top instead of wood.
