I have several white signs for a cave on my map.
Should they become gray when I have looted/found everything inside?
Is it mandatory to find 100% of all possible items inside the cave or do I have to find at least the main treasure chest?

According to the Witcher Wiki, it's 10% per adrenalin point.
Effect Adrenaline Points also increase your Sign damage. 10% Sign Intensity per Adrenalin Point.

As for your setup, it's definitely ideal, however it's not the MAX sign intensity percentage. Other players have reached over 400% with the use of a optimal build and Foglet Decoction (only effective during clouded weather look here for ingredients to craft one. But that's not a constant boost to your sign intensity, but it does prove to be a significant plus.
If you're looking to get maximum sign intensity with no conditions, abilities, or temporary effects, your build is excellent.

Entrance markers do not appear to grey out. Even caves that have a quest in them, such as the cave at the Clan Tordaaroch Forge, which has a very clear "main chest," doesn't grey out when "completed."
