In Mists of Pandaria, Blizzard have added a Black Market Auction House to the game, in order to purchase items that are otherwise hard to get or no longer available by normal methods in game.

Where in the game world is the Black Market Auction House located, is it faction specific, and are there any requirements in order to be able to use it?

You'll need the MoP expansion. The Veiled Stair is a level 87 area.

The BMAH is located within the Veiled Stair area, a northern subregion of the Valley of the Four Winds. A bit to the south-east of the Tavern the Mists you will be able to find Madam Goya, who is the leader of the BMAH.

You need a lot of gold, as the lowest bids start at 10k. The area is not a safe zone, so you can expect a lot of ganking on PvP realms.

If I remember correctly, the deposit gets returned to you when something sells, but the auction house itself takes 5% out of the winning bid as it's fee. I do know that it's a fixed portion, and not affected by duration.

The Neutral Auction house takes a larger cut -- 15%. Filthy goblins.

As a reminder, you don't get your deposit back if your item doesn't sell. The cost of the deposit itself varies with duration (as you noticed) but the vendor price of the item in question as well.

Deposit (12hr) = 15% of Merchant Sell Value
Deposit (24hr) = 30% of Merchant Sell Value
Deposit (48hr) = 60% of Merchant Sell Value

Again, the goblin-run Auction Houses charge more:

Neutral Auction Houses:
Deposit (12hr) =  75% of Merchant Sell Value
Deposit (24hr) = 150% of Merchant Sell Value
Deposit (48hr) = 300% of Merchant Sell Value

World-of-warcraft – What happens to unsold items on World of Warcraft Auction House

If you cancel an auction or one of your auctions expires, you will get the item back via in-game mail. There, it will stay for (I think) 30 days, if you do not pick up the item from the mail by then, it will be irreversibly lost.
