Using the standard Salesforce interface for Tasks (Page Layout), there is a related list for Attachments. However, there's no button to add a new Attachment to an existing Task, and when I edit the Tasks Page layout the Attachments related list is "not customizable".
Is there another way to add an attachment to a Task (or Event)? I'd like to make sure I'm not missing anything using standard customization before using Apex/Visualforce.
Here is my POC to show that winter '14 don't have such issue like On inserting an Attachment from UI doesn't fire the trigger.
I create the trigger and Apex class from the sample you provided and then inserted an Test Attachment file from UI on a record.

Debug: Debug logs created for attachment trigger and System.debug lines were there.
Attachment has been renamed as "test"
However on attachment upload notification detail page it doesn't show the name changed (even trigger got fired). It shows changed name (as sample trigger did) in related list.
There were no issues in trigger, may be this issue has been solved. Hope this help.
For the Note case, /002/e?parent_id=[record id] doesn't work for me with the 18 digit id (Data Not Available) but does with the 15 digit id.
For the Attachment case, /p/attach/NoteAttach?pid=[record id]&retURL=... works, though as an even less "documented" URL pattern you may not want to risk it. 15 digit id again.

You just need to Edit the task itself to see the Add Attachment button. It's only available in the task edit view.
