Multiple editions of Civlization 5 have been released.
Some as retail boxed versions, others as digital downloads.
The premium editions contain bonuses which range from:
- metalic figurines
- art work
- music tracks
- behind the scenes videos
- extra civilizations
- map packs
- posters / misc. memorabilia
Many retail stores provided different or unique perks included in the list above, so it is difficult to give a full list. Also it is not possible to buy a supreme version with all the perks listed above.
Note that There is no hard copy manual released with any versions of Civlization 5, further more there are no hard copies of the technology tree.

Included in the retail copies is a small quick-start manual to get you up and running as quickly as possible.
The last sentence references a small pamphlet included in retail copies, but not the full manual.
What you may have seen on youtube was the Collectors edition artwork book
This is not the manual, but a picture book containing artwork related to the game.
As far as I know, this edition was only produced as a once only batch when the game was released and it is quite difficult to find copies for sale.
Did you configure it to be arid and old; or random for everything and just have a run of similar worlds?
You could explicitly set the World Age and Rainfall (from your description, the temperature could have varied a bit) to mix it up.
- World Age: Adjusts the map "smoothness", as a longer world has eroded more.
- 3 billion years: highly mountainous
- 4 billion years
- 5 billion years: flat
- Temperature:
- Cool: more ice, more tundra at lower latitudes
- Temperate
- Hot: more jungles, less tundra
- Rainfall:
- Wet: more forests and jungles
- Normal
- Arid: more deserts
Configure the settings to a younger earth (3 or 4 Gyr) and temperate or wet rainfall if you want to get away from what you're describing. You can do so on the "Advanced Setup" panel when you configure a game.

I made an experiment with two knights both having medic1 + medic2 + fountain of youth upgrade. I then moved one of them on top of a mountain inside my territory and the other one next to it on a non-mountain tile. The knight landing on the mountain takes 50 damage and ends it's turn at 50 ( 100 - 50 ). Then it heals 40 before the end of next turn and takes another 50 damage after the end of turn bringing the total to 40 (50 + 40 - 50). Thus the unit seems to loose 10 hit points per turn. I have no idea why the unit heals only 40 points per turn. Perhaps fountain of youth promotion and medic promotions do not stack at all.
