I found a "Twists of Fate" card in the "Special Delivery" chest at Gorhart – I assume for having played the demo. I dismissed it pretty quickly, but I seem to remember it being titled "Confident."
However, "Confident" is not listed as one of the cards in the Destiny menu. I also dug through my inventory and couldn't find it there.
How do I activate this card?
The problem is probably that you're trying to change the resolution while your character is loaded, instead of exiting back to the main menu and doing it from there. For some reason, once you're already playing your character, the option to change your resolution disappears.
There are two ways to set the difficulty in KoA.
Start a New Game
When you start a new game, you get to pick the difficulty.
Realistically, you're not very likely to abandon your game every time you want to change the difficulty
From the Options Menu
Go to the pause screen, then the Options menu. The difficulty controls are under gameplay options, which you can adjust at your leisure.
There do appear to be some conditions as to when you're allowed to change the difficulty (it might be as simple as, "cannot change while in combat", but I haven't been able to verify when it's disabled).

Twists of Fate are permanent bonuses, so you don't need to activate it. You can view your Twists of Fate by going to Status -> Twists of Fate
