For some reasons no cooldown items go on cooldown when I hit assigned the hotkey. I noticed
that my potion and wards do go off, but items like blade of the ruined king, hextech gunblade, etc. are not going, when I try to use their active. I am using smart cast by the way. But this just started happening recently so I don't think it's smart casting that is affecting my problem.

An ability is placed on cooldown relative to the CDR you had at cast time. Buying a CDR item (or entering a CDR aura, like Soul Shroud) will not impact the remaining cooldown time on abilities currently on cooldown. They will impact any future casts thereafter, however.
(Source - "Cooldown reduction is determined at the moment an ability is activated. Increasing/decreasing your cooldown reduction while the skill is on cooldown will not affect the cooldown.")
As of today, 3 items give the most cooldown reduction, which is 20%:
65 Ability Power, 50 Attack Speed, 10 Mana Regeneration. UNIQUE Passive: +20% Cooldown Reduction.

75 Ability Power, +20% Cooldown Reduction, 12 Mana Regeneration. UNIQUE Passive: Dealing magic damage to an enemy champion below 40% health inflicts Grievous Wounds on them for 4 seconds. (Grievous wounds reduces healing and regeneration rates by 50%)

90 Armor, +20% Cooldown Reduction, 400 Mana. UNIQUE Passive: Reduces the attack speed of nearby enemies by 20% (1000 range).

This is a known bug. Items that have an active on a champion (such as DFG active, Face of the Mountain active, and bork), sometimes do not activate on smart cast.
A temporary fix would have to be clicking the target, sorry.
