Diablo – How exactly does Hwoj Wrap work with Bane of the Trapped

As I understand it Locust Swarm snapshots on cast, meaning it takes your stats and buffs at the time of cast and will not update new changes until the next cast.

If I have a Hwoj Wrap, this makes my Locust Swarm slow enemies.

If I have Bane of the Trapped I do bonus damage to enemies that are crowd controlled. This includes slow effects.

If I cast Locust Swarm while wearing Hwoj Wrap and Bane of the Trapped, does this initial cast apply the Bane of the Trapped's extra damage or would I have to cast Locust Swarm twice, once to apply the slow, and then again to snapshot the extra damage the slow grants?

After testing this myself I find that the spell Locust Swarm does not snapshot on cast in the manner that I previously thought.

To test this I dismissed my follower and all pets, and used ONLY Locust Swarm to damage my enemies. The very first tic of the dot applied by Locust Swarm was always lower than the subsequent tics, and every tic other than the first was always the same amount of damage.

This tells me locust swarm does its normal damage with the first tic and applies its slow in the same action, then the subsequent tics benefit from the damage increase from Bane of the Trapped.

Yes, dodge simply gives to attackers a x% to miss their attacks. It works on all attack types:

Melee Attacks
Ranged Attacks
Ranged Spells
Ground-based AoE

Dodge does not work only against status effect modifier like fear, jail, etc.

Dodge is linked to dexterity and seems not hard-capped but its formula has a diminishing effects like explained in this answer.

2000 Dexterity, for example, provide you a 40% dodge:

100*0.100 + 400*0.025 + 500*0.020 + 1000*0.010 = 40%

Did some testing after you asked this. The tests went as followed:

Method: I went outside of New Tristram and used the Hex ability (unruned) against the zombies there. I unequipped and reequipped INT and Attack Speed items, as well as my weapon to test if there were any changes in duration or rate.

Findings:

In a group of enemies the Fetish Shaman will:

Hex 4 targets over his 12 seconds active
Not Hex Targets already Hexed
Hex targets while other targets are still Hexed (other targets will remain Hexed for the full duration as well).

With a single target the Fetish Shaman will:

Hex the 1 target 2 or 3 times over his 12 seconds active.
Will only Hex the target if the target is no longer Hexed.

Running several tests with a Stopwatch on my Android I found that:

The single target was Hexed for around 5.5 seconds.
The Fetish Shaman will Hex a random target in a group of mobs around every 3 seconds.

Additionally, changing my stats around (adding Attack Speed and INT, and changing my weapon) appeared to have no effect on how long the Fetish Shaman is active or the duration the target is Hexed.

Given these tests, I believe that there is no way to increase how long the Fetish Shaman is active and how often he turns an enemy into a chicken, and that there is no way to increase the duration of the chicken effect.

I'd also like to note that there can be a short delay between the Fetish Shaman spawning, and then using the polymorph ability. Which is why he may only Hex a single target 2 times instead of 3. 5.5s * 2 = 11, so if the Fetish Shaman takes a second to cast on spawn he'll miss the chance to polymorph the target a third time before he despawns.
